How to find your formation document
Your formation document is the state filing that created your LLC. It may be called Articles of Organization, Certificate of Formation, or Certificate of Organization.
Where to look

Check your state filing portal
Sign in to the state website where the LLC was formed and look for the filed or approved formation document.

Ask your registered agent
If someone formed the LLC for you, your registered agent or formation service may have a copy.

Search your email and files
Try searching for “Articles of Organization,” “Certificate of Formation,” or the LLC’s legal name.
What to upload
- The filed or approved state formation document.
- A PDF, PNG, or JPEG file.
